As a church, we are taking an intentional step to better understand and strengthen the spiritual health of our congregation. The goal is not to measure or grade individuals. No personal scores will be shared or tracked. Instead, the combined responses will help guide future teaching, discipleship pathways, ministry development, and leadership decisions as we pursue the vision God has given us as a church. Your responses are completely anonymous and confidential, so we encourage you to answer honestly and thoughtfully.

What is the Spiritual Health Survey?
The Spiritual Health Survey is a gentle diagnostic tool designed to help measure your progress on the discipleship pathway. By looking at our daily habits and our closest relationships, we can see how our private faith is transforming our public lives. It invites you into the adventure of experiencing the presence of God in your everyday life.
Why are we taking this survey?
Simply put, we want to evaluate how well we are doing in the things we say we are doing. We have designed this survey around the core pillars of our church: the 7 Rhythms from the 10-Week Discipleship Journey (the banners hanging on the wall in our auditorium), the 9 traits from our recent Spiritual Friendship series, our Ten-Year Vision, and our church tagline: "Encouraging faith. Inspiring purpose". This helps us see if we are effectively equipping you for the journey!
Is the survey anonymous?
Yes, it is completely anonymous. We do not ask for your name, email, or any contact information to take the survey. We want you to feel completely comfortable providing honest, vulnerable answers about your spiritual journey.
What will leadership do with the results?
First, our executive staff will review the overarching results to get a 30,000-foot view of the church's spiritual health. Then, we will break the data down into specific "Spoke Reports" for our various ministry teams (such as Men's Ministry, Women's Ministry, Small Groups, etc.). This allows our ministry leaders to be actively involved in the process—reviewing the data through the lens of their specific ministries, praying over what we glean, and making tailored adjustments to help their teams grow.
How long will the survey take to complete?
The survey is brief and should only take about 5 to 10 minutes. It consists of 14 core statements where you rate your daily habits on a 1–4 scale, a few quick demographic checkboxes, and optional short text boxes to share your volunteer roles or current hurdles.
